Don’t miss NYSHIP’s annual open enrollment period, which is happening right now, and continues until the end of this month (November 30th). Here are some changes you can make to your plan:

  • Enroll in coverage of add eligible dependents to coverage without 30-day waiting period usually applied to late enrollment (effective date the enrollment form is received by Scott Voorhees*, Rm. 7301.5)
  • Change from family to individual coverage, without providing proof of other health coverage, even if your deductions are taken out pre-tax (change effective first full pay-period of 2015).
  • Cancel coverage, without providing proof of other health coverage (cancellation effective as of first full pay-period of 2015)
  • Change from pre-tax to post-tax status or vice versa (change effective first full pay-period of 2015).

From Leslie E. Williams, University Executive Director of Shared Services:

  • “Please note that employees enrolled in pre-tax status have deductions taken prior to their paycheck being taxed but are limited in the changes they can make during the year. Employees enrolled in post-tax status are able to change from family to individual coverage or cancel their coverage at any time”
  • “If you are participating in the Annual Open Enrollment Period, you must complete and submit the Health Benefits Enrollment form (PS-404)”  Form found here:  http://www.cs.ny.gov/forms/ps404.pdf.

Eligible dependents you may enroll:

  • Spouse or domestic partner
  • Dependent children (under 26 years of age)–includes natural children, adopted children, dependent step children
  • Disabled dependents (26 years of age or older).
