As you progress through the program there are certain times that you need to file paperwork with the Theatre Office. Copies of these forms are available here to make them easier to access.
Second Exam Forms
Once you pass the first exam, you will need to work with an advisor to put together your Second Exam committee and begin writing your field statements. Your completed field statements must be submitted to the Executive Officer with the accompanying Field Form. Then you will begin constructing the booklist for each field, which must be submitted with the Booklist Form.
